The Dangers of False Negatives in Drug Testing

In drugs of abuse testing there are several criteria in evaluating a testing device.

- Speed of Test
- Accuracy
- Ease of Use
- Specificity & Sensitivity
- Flexibility in Testing Menu
- Cost

In comparing devices using these criteria, it becomes very clear that not all devices are created equal. While the weight that each of these criteria carries may vary from customer to customer, accuracy is always at the top of the list.

Most of our customers are familiar with the term and consequences of 'False Positives'. False positives result when a drug test screen indicates a positive result, but GC/MS confirmation indicates a negative result. Fewer of our customers are aware of negative consequences of 'False Negatives'. False negatives result when the drug screen yields a negative result, and therefore, the specimen is never sent to lab for GC/MS confirmation and drug use is never detected. False negatives can result when screens are too conservative or cut-off level thresholds are too high. The goal in developing the best, most accurate POCT devices is to maximize sensitivity and minimize false positives.

Low positivity rates could suggest that there is a need to modify the drug testing panel. If positivity rates for drug screens are below 15% in the corrections market, we encourage our clients to examine which drugs are yielding positive results, and which are not. Changing the drugs tested within the panel or expanding the panels like OPI to include tramadol, fentanyl, and buprenorphine often help to decrease false negatives and uncover drug use that was previously undetected.
